Special Meal Accommodations

If you are needing meal accommodations, please complete the "Medical Statement" form, it must be signed by a physician.  If you simply desire a substitution for milk, please complete the "Milk Substitution" form.  Once completed, please give to your school site's Lead Kitchen worker and also your site's health tech or the Snowline District Nurse's office.  They will be forwarded to Terri Nelson, Operations Supervisor at our Central Kitchen for implementation.
