Educational Benefit Form

Please fill out the EDUCATIONAL BENEFIT FORM.  All families are required to complete a form for the 2023/24 school year.

 

By filling out this form, you help ensure that your student's school continues to get the full educational funding needed for your student's education, which includes:

 

Computers

Books

School Supplies

 

Copies of Educational Benefit Forms, or previous year's applications, are not accepted. Educational Benefit Forms may not be faxed as we are scanning EBF this year and cannot scan a faxed copy.

If completing a paper Educational Benefits Form, you can turn them into either your school site office.
